Knoch students wishing to take part in school athletics must have a physical examination and return completed PIAA forms to the Athletic Office as soon as possible before the season starts, preferably no later than 1 week prior to the first day of mandatory practice. PIAA form Section 5 ( Health History - you or your physician complete) & Section 6 (Physical Form to be completed by a physician) are required for students participating in a sport for the first season in a given school year. The Section 7 (Recertification) is to be completed by students who will be participating in the second or third season in a given school year. A "Yes" answer for questions on the Section 7 form will require a completion of Section 8 to participate in the second or third season of the school year.

The PIAA forms are part of the Big Teams Student Central registration process with the exception of section 6. This must be printed, signed by a physician and uploaded to your Big Teams Student Central account. Physical forms must be dated after June 1 or later to be valid for participation in the current/ upcoming school year. Please call the athletic office at ext. 4800 with any questions.
